The Baby Immune System and How It Develops in the First Year

Having a new baby can suddenly make you notice every coughand sneeze.


A family member says, "It's just allergies," and suddenly you are doing the mental calculation of how badly you actually need them to hold the baby today.


It is funny how quickly germs become something you think about once you are responsible for this tiny person.


  • But what is actually happening with your baby's immune system during that first year?

  • Are newborns basically defenseless?

  • When does a baby's immune system get stronger?

  • And do babies really need to get sick to build immunity?


Something to keep in mind is that is not one magical age when it suddenly becomes "strong." Those changes happen gradually throughout infancy and continue well beyond the first birthday.


How Does Baby's Immune System Change Over the First Year?


When we hear that babies have "weak immune systems," it can sound like they have almost no protection at all. But their immune system is already working from birth, just with a lot less experience than an adult one.


Some defenses are built in, while others become more experienced as the body learns to recognize and respond to germs.


There are not specific milestones where something suddenly switches on at exactly 4 months or 8 months. Instead, several changes are happening at the same time throughout that first year.


Some protections your baby started life with gradually fade as they grow and their own responses become more capable, and their exposure to the outside world increases dramatically.


Birth to 2 Months: Baby Gets a Head Start


Before your baby is born, certain antibodies pass from you to your baby through the placenta.

Antibodies help the immune system recognize and fight particular germs. The protection your baby receives during pregnancy gives them some extra support during those early months while their own defenses are still immature.


This is temporary, though.


Those antibodies gradually decrease after birth, and they do not protect your baby from every illness.

Your newborn gets a helpful head start, not an invisible force field.


Breast milk also contains antibodies and other components that can provide additional protection against some infections. This is one of the benefits of breastfeeding, but I think this is also an area where we need to be thoughtful about how we talk to parents.


Breastfeeding is one source of immune support. It is not a test you pass or fail.


A formula-fed baby's immune system is still learning and maturing normally. Feeding your baby formula does not stop their body from building its own defenses.


2 to 4 Months: Building More Protection of Their Own

As the first few months pass, some of the temporary protection your baby received during pregnancy begins to decrease. Meanwhile, their body is responding to an entire world that is brand new to them.


This is also when routine childhood vaccines commonly begin providing protection against specific serious illnesses. Vaccines help the immune system learn to recognize certain diseases so it is better prepared if it encounters them later.


They are one part of the bigger picture of protecting your baby while their body continues maturing.


4 to 6 Months: Older Does Not Mean Invincible


By this point, your baby's immune system has had several months to grow and respond to life outside the womb. And it is very different from the brand-new system you brought home from the hospital.


But getting older does not mean your baby suddenly stops getting sick.


The temporary antibodies they received before birth continue to decline, and their own defenses still have plenty of maturing to do.


There is also another change happening around this time. Your baby is becoming much more interested in the world. Which means they are about to touch a lot more of it. (If they aren't already.)


6 to 9 Months: Everything Goes in the Mouth


Somewhere around this stage, the baby who mostly stayed where you put them may turn into a determined little investigator.


They are grabbing toys, eating solid foods, touching the floor, sitting in restaurant high chairs, playing near other children, and putting an impressive number of things directly into their mouth.

It is normal development, but it also creates more opportunities to come into contact with germs.


Add childcare, siblings, playgroups, errands, or simply more time outside the house, and your baby's world can expand quickly. So if your 8-month-old suddenly seems to catch more illnesses than they did as a newborn, it does not necessarily mean anything has gone wrong.


They may simply be coming into contact with a lot more people, places, and things.


9 to 12 Months: A Much More Experienced Immune System


Think about how different your almost-one-year-old is from the newborn you first brought home.


The baby who once stayed exactly where you placed them may now be crawling through the house, stealing food from your plate, opening cabinets, and somehow locating every tiny object you missed while vacuuming.


A lot has changed inside their body too. By the end of the first year, their immune system has had months to respond to everyday exposures and build protection against some specific illnesses.


But the first birthday is not a finish line. Your baby's defenses will continue maturing throughout childhood. They are simply much farther along than they were on day one.


Does Your Baby Need to Get Sick to Build Their Immune System?


You may hear:

  • "They need germs."

  • "It's good for babies to get sick."

  • "That's how they build immunity."


There is an important distinction here. Your baby's body is constantly interacting with the world around them, but that does not mean they need to intentionally catch infectious illnesses in order to develop normally. Everyday life already provides plenty of exposure.


You do not need to sanitize every toy the second it touches the floor, but you also do not need to hand your baby to someone with a hacking cough because "getting sick is good for them."

Both things can be true and babies can explore the normal world around them while we still reduce exposure to illnesses when we reasonably can.

The newborn you brought home and the almost-toddler celebrating their first birthday are incredibly different. During that first year, your baby's body has been busy learning how to respond to the world around them, even though there is still plenty of growth ahead.


There is so much happening inside your baby that you cannot see just by looking at them.
